You know the risks exist. The question is whether you see them before they strike.
Structured risk assessment with risk scenarios, consequence analysis, probability and occurrence. Visual risk matrix showing the complete risk picture. AI assistance that helps you formulate and improve.
From startups to publicly traded — risk management that fits everyone.
How risk assessment works in AmpliFlow
Move risks around the matrix and watch risk scores change in real-time. Risk Score = Impact × Max(Probability, Occurrence). This is just a taste — in AmpliFlow you also get AI assistance, process linking, and detailed status tracking.
Screenshots from AmpliFlow: 1) Risk table with all columns (risk number, scenario, consequence, probability, occurrence, risk score, status). 2) Risk form where you grade impact and probability. 3) Optionally the D3 chart with gradient background.
See SALMAN.md for asset guidelinesFrom Excel graveyard to living risk register
Most organizations have a risk register. The problem is that it lives in a spreadsheet that nobody opens until the auditor asks for it.
The risk register in Excel
- Updated once a year — the days before the audit
- Nobody knows which version is current
- Risk scores calculated manually — if at all
- Hard to track status and responsible persons
- Management never sees the complete risk picture
Structured risk register
- ✓ Risks updated continuously with clear status (8 steps)
- ✓ One version, always current — no version chaos
- ✓ Automatic risk score based on impact, probability, and occurrence
- ✓ Responsible person, target date, and estimated risk reduction per risk
- ✓ Visual risk matrix gives management overview
What you actually fill in
Each risk in AmpliFlow follows a structured template that makes assessment consistent and traceable.
Risk scenario
Describe what could happen. AI assistance helps you formulate clear scenarios based on your industry and previous risks.
Potential consequences
What are the consequences if the risk occurs? AI can suggest consequences based on the scenario you've described.
Impact rating
Assess the severity of the consequence on a scale of 1-5. With multi-dimensional assessment, you can evaluate from multiple perspectives (financial, environmental, safety).
Probability
How likely is it that the risk will occur? Scale 0-5, from "Not applicable" to "Likely to occur multiple times per month".
Occurrence
Have similar events occurred before? Scale 0-5, from "Not applicable" to "Occurred more than 6 times in the last 12 months".
Risk reduction measures
Describe what you're doing to reduce the risk. Specify responsible person, target date, and estimated probability reduction.
One risk register. Multiple perspectives.
With multi-dimensional impact assessment, the same risk can be evaluated from different perspectives. Create your own categories that fit your business — and the auditor sees you're in control.
Your risk register
A single source with all risks. Assess the consequence from each perspective and get a combined risk score.
Quality
How does the risk affect customer satisfaction, product quality, and delivery precision?
Environment
What environmental impact does the risk have? Emissions, resource consumption, waste?
Information Security
Does the risk threaten confidentiality, integrity, or availability of information?
Occupational Health
Could the risk lead to illness, injury, or hazardous working conditions?
AI that helps you formulate — not think for you
Three fields in the risk form have built-in AI assistance. You write, the AI suggests improvements or additions based on your previous risks and industry context.
- Risk scenario — Generate or improve the description of what could happen
- Potential consequences — Suggestions for consequences based on the scenario
- Risk reduction measures — Suggestions for how to reduce the risk
The risk matrix that shows the complete picture
See all risks on an interactive map. The X-axis shows updated risk score (after planned actions), Y-axis shows probability. Size shows original risk, color shows current state.
- Animated visualization showing the effect of risk reduction measures
- Click on a risk to open the form directly
- Color coding: green (below 7), yellow (7-14), red (15+)
- Interactive tooltips with complete information about each risk
Risks linked to your processes
Each risk can be linked to specific process steps in your process map. Highlighted risks are visible directly in the process map so employees see where the risks are.

Questions about risk management
How is the risk score calculated?
Risk Score = Impact × Max(Probability, Occurrence). Probability assesses how likely the risk is to occur in the future. Occurrence assesses whether similar events have happened before — in your organization or industry. AmpliFlow uses the higher of these two for the calculation.
What's the difference between probability and occurrence?
Probability is about the future: 'How likely is this to happen?' Occurrence is about the past: 'Has this happened before?' The scale goes from 0 (not applicable) to 5. For example, a risk might have low probability (new security systems in place) but high occurrence (happened several times last year).
Can we assess consequences from multiple perspectives?
Yes. With multi-dimensional impact assessment, you can create your own consequence categories — financial, environmental, occupational health, information security, reputation. Each risk gets a rating per category, and the highest is used for the risk score.
How do we track risks over time?
Each risk has a status field with 8 steps: Identified → Actions initiated → 25% complete → 50% complete → 75% complete → 100% complete → Managed → Pending. You also specify estimated probability reduction and see an updated risk score based on planned actions.
Do you support ISO 31000?
Yes. AmpliFlow follows ISO 31000 principles and supports the risk-based thinking required in ISO 9001, 14001, 45001, and 27001. The risk register documents scenario, consequence, assessment, and risk reduction measures — exactly as the standards prescribe.
Can we import risks from Excel?
Yes. Bulk import and edit risks via an Excel-like interface to get started quickly or make larger updates. You can also archive outdated risks without losing the history.
